Medications
Please bring a list of all your medications to each appointment including the name, dose and schedule of prescription, over the counter, herbal medications and supplements.
Imaging
If you have had an X-ray, CT Scan, MRI or other tests performed as a result of your current medical symptoms, bring the test results with you.
Insurance Cards
At each visit, we will copy your insurance card as it contains information required by your insurance company to file the claim.
Referral
If your health plan requires a written referral to see a specialist, please make sure we have received referral prior to your appointment date.
Co-Pay
Most health plans require patient co-pays for each office visit. We are required by your insurance company to collect the co-pay.
